
Braybrook
Mambourin Braybrook has been providing services to people with disabilities for over 40 years. Today over 80 service users of varying ages and abilities attend the site to learn new skills, participate in leisure activities, build and develop social networks and participate in the broader community. As part of the regular Person Centre Planning (PCP) process, individuals, together with their key representatives, identify the types of goals, priorities and activities most relevant and important to the person at their current stage of life. Service arrangements are then structured to accommodate each individual’s goals and priorities as identified in the PCP with services being delivered in group program activities, individualised options or a combination of both.
People who choose an individualised service arrangement are supported in directing and guiding their formal and informal supports, to achieve their aspirations and goals. Some of our service users have chosen to utilise their funding to provide them with 1:1 support in group program activities, other service users are participating in community volunteering or are being supported in flexible employment placements of their choosing.
Most service users attending Braybrook have chosen to participate in the group program model, whereby adults with similar interests are supported in achieving their personal goals in a group setting. Program offerings are grouped according to five key curriculum areas, being academic skills, life skills, recreational and leisure pursuits, work training opportunities and the arts.

As part of our quality system regular assessments of client outcomes are conducted. Outcome reports are reviewed during the PCP meetings and are also available upon request.
Braybrook is also home to a new transitional program “Get Ready”.
The objective of this program is to offer a service that:
1. Focuses on a younger age group (18-25 years of age).
2. Transitions adults slowly into an adult environment.
3. Creates a broad range of opportunities for people to explore their options and future directions.
The Get Ready program is based on a set curriculum which allows participants to develop their general life skills, understand different options open to them and to map out future pathways to achieving their goals and life aspirations.
